The current poster presentation examines the LaMia Flight 2933 plane
crash by using the Haddon Matrix. In 29th November 2016, the passenger plane,
which moved from Santa Cruz de la Sierra (in Bolivia) to Medellin (in
Colombia), fell near the mountainous terrain near Medellin; and it was
transporting 77 passengers including the Chapecoense football team from
Brazil’s First Football League teams and the aircrew. This accident in which 71
people died was referred to as the accident that destroyed a football team. The
world of sport was saddened by the plane crash, which the Chapecoense football
team encountered when it would be playing at Copa Sudamericana Final with the
Atletico Nacional football team. Only six people survived, which three of them
were footballers.

 

The Civil Aviation Agency, which has completed its investigation of the
accident, announced that the accident was due to the end of the aircraft
fuel. It was stated that the accident was not a sabotage or technical error,
but it occurred completely due to a human-caused mistake. It was revealed that
the pilot both did not take fuel on the plane and reported the emergency late.
It was also underlined that LaMia which is Bolivian-based airline was not be
appropriate for the international flight standards.

The Brazilian club planned to fly directly from Brazil to Colombia at
the beginning, but this plan was rejected. The budget of Chapecoense Club did
not allow direct flight with another aircraft. It was decided on the current
route. The flight distance between Santa Cruz de la Sierra and Medellin is 1605
nautical miles which means 2972 kilometers. On the other hand, according to
official technical reports, a standard British RJ-85 type aircraft which
designed for short and medium distances can take 1600 nautical miles (2960
kilometers) with maximum fuel.

 

The pilot of the 2933 requested priority for landing to Jose Maria
Cordova airport control tower due to fuel problems, but there was another plane
which made an emergency landing. The priority emergency landing permit was
given to another plane, and it was said to stay in 21 thousand feet for the
aircraft carrying Chapecoense football team. The airplane, which circled over
about 15 minutes, wanted to increase its range by decreasing speed and
altitude, but eventually it started to experience electronic problems. The
captain of the plane repeated the emergency landing request and the tower
allowed for this time. However, the plane which experienced power problems fell
into 22 kilometers south of Medellin, 17 kilometers from the airport.

hours and 3,417.41 total RJ85 flying hours.

Co-pilot
factors: The 47-year-old captain obtained his license
from Airline Transport Pilot (ATP) and his medical certificate was reported as
1st Class. His last proficiency check was done in 3rd
July 2016. According to LAMIA status documents, he had 6,923.32 total flying
hours and 1,474.29 total RJ85 flying hours.

Passenger injuries are demonstrated in the Table 2. 70 passengers who
had fatal injuries died at the site of the accident. Six passengers got out
alive from the accident.

The manufacturer of the aircraft is BAE and the model of the plane is
146 AVRO RJ-85. The plane had 21,640:45 total flight hours.
